Should underwear sit over or under an apron belly?

Over or under both work. The crease doesn’t. A waistband that lands in the fold gets pushed down by the weight above it, rolls, and traps heat against the skin. People with an apron belly split into two camps: high-waisted briefs that clear the fold, or low and under-belly styles that stay fully beneath it.

An apron belly (pannus) is graded 1 to 5 by how far the fold hangs, from the pubic hairline at grade 1 down toward the knees at grade 5.^[1] The further it hangs, the harder it is for any waistband to get over the top, which is why the two camps exist.

• Over the fold. A full or high-rise brief pulled up to the natural waist. One Reddit user said underwear under the belly “drives me insane, so I absolutely need it to go up”.^[3] This works best on grade 1–2 folds, where a waistband can still reach above the crease.
• Under the fold. A low-rise or maternity under-belly brief that sits beneath the crease. The fold rests on your skin, not on elastic. Pair it with a cotton liner so skin doesn’t sit directly on skin.
• High-waisted shorts. A third option some users like, because longer legs “don’t bunch up weirdly” with an apron belly.^[3]

What to look for in apron belly underwear

Look for four things: a size run that goes past 3X, a waistband height that avoids the crease, cotton where skin meets fabric, and a return policy that lets you try a size. The last one matters more than usual, because letter sizes don’t account for a fold and one brand we checked takes no returns at all.

1. Size range. Of the five pairs we verified, the largest sizes are 4XL (Hanes and Knix), US 24 (M&S), EU 50 (Miss Mary) and a numeric 17 with no chart (Dry Apron). Measure your hips around the widest point, fold included, and use each brand’s chart.
2. Rise. High rise if you go over, low rise if you go under. Mid rise is the one to avoid.
3. Cotton at the skin. Heat, moisture and friction are the main drivers of intertrigo, the rash that forms in skin folds.^[2] A pure-cotton brief, or one with a cotton pocket or panel for the fold, keeps a synthetic layer off the damp skin.
4. Returns. Buy one pair first. Miss Mary allows 100 days. Dry Apron “does not accept returns or give refunds”.

What people with an apron belly say about underwear

In the seven underwear comments we collected from apron-belly threads, the most common theme was waistband position: people pull their underwear high over the fold, or switch to under-belly and maternity styles. Only one brand we verified, Marks & Spencer, was named directly. Rolling down under the fold was the main complaint.

These are individual comments, not a survey. One user who had a tummy tuck said the change was that their underwear no longer rolls “under my big apron belly”.^[3] That tells you how common roll-down is; it isn’t an argument for surgery.

Underwear can’t treat or prevent a rash under an apron belly, but it can avoid making one worse. • Maternity under-belly briefs: recommended on Reddit for an apron belly, but the plus-size under-belly styles we found were marketplace listings we couldn’t verify on a brand page.
