Surgeons grade it 1 to 5 by how far it hangs. Diet and exercise can shrink the fat but not the skin, so day to day, what helps most is the right garment, a dry fold, and knowing when surgery might be covered. An apron belly is excess skin and fat that hangs down from the abdomen below the waistline, like an apron. Doctors call it a panniculus or pannus. StatPearls defines it as “an apron of excess skin and fat hanging from the abdomen below the waistline”. It’s also called a belly apron, a hanging belly, a mother’s apron or a pannus stomach. “Apron belly” isn’t a clinical term, but insurers use the same image: Anthem’s policy says “this loose skin is called a pannus or apron”. Two details matter for everything else on this page: - **It’s skin as well as fat.** The surgery that removes it is billed as excision of “excessive skin and subcutaneous tissue”. Fat responds to weight loss. Stretched skin mostly doesn’t. - **It isn’t always just the lower belly.** A panniculus is most often on the lower abdomen but can include the mons pubis, upper abdomen, flanks and back. ## The 5 apron belly grades Apron bellies are graded 1 to 5 by where the lower edge of the fold hangs. Grade 1 covers the pubic hairline, grade 3 covers the upper thigh and grade 5 reaches the knees. The grade matters because symptoms, garment choice and insurance decisions all change with it. 1. 1 Grade 1: the fold covers the pubic hairline and mons pubis, but not the genitals. 2. 2 Grade 2: the fold covers the genitals and the upper thigh crease. 3. 3 Grade 3: the fold covers the upper thigh. 4. 4 Grade 4: the fold covers the mid-thigh. 5. 5 Grade 5: the fold covers the knees and below. (Obesity Surgery, 2000). In a 2000 study of 577 panniculectomies that used this grading, people with grades 3–5 “suffer more back-pain and problems of hygiene”. Insurers also use the fold’s position: Cigna, Aetna and Anthem all require it to hang at or below the pubic bone before they’ll consider covering removal. Grades 1 and 2 are worded slightly differently in some papers. We use the American Society of Plastic Surgeons’ wording. ## What does an apron belly look like? An apron belly looks like a soft fold of skin that hangs down over the front of the pubic area, with a crease underneath where it meets the lower body. You can usually lift it, and skin touches skin in the crease underneath. It differs from a round belly, which sits forward and doesn’t fold over. An apron belly forms when abdominal skin stretches and loses elasticity. That usually follows weight gain, large weight loss, pregnancy or aging. When the skin can no longer spring back, gravity pulls the skin and the fat under it into a fold. - **Weight gain.** “When you carry extra weight, skin and tissue stretch… This stretching causes skin and tissue to lose elasticity.” - **Large weight loss.** Insurers’ policies describe the pannus as something that “often occurs following massive weight loss”, including after bariatric surgery. Weight-loss medications are part of this now. In the ASPS 2025 statistics, more than four in five member surgeons reported GLP-1-related consultation requests, and 67% named panniculectomy among them. - **Pregnancy and aging.** Loose abdominal skin is linked to “massive weight loss, postpartum changes, and aging”. - **Body type.** How the fold forms varies with “body type, their fat deposition pattern, and the amount of weight gained or lost”. ## Problems an apron belly can cause A larger apron belly can cause rashes and infections in the fold, back and joint pain, trouble walking, and difficulty with hygiene. The American Society of Plastic Surgeons lists impaired walking, chronic back, neck and shoulder pain, intertrigo, infections and skin breakdown. The most common day-to-day problem is **intertrigo**, a rash where skin rubs on skin. It’s driven by “warm temperatures, friction, moisture, maceration, and poor ventilation”, and yeast (Candida) can infect it. A heavier fold can also strike the thighs when walking. Weight loss can shrink the fat in it, garments hold and smooth it day to day, and good skin care prevents the rash. A tummy tuck is almost always treated as cosmetic.
